Charlotte Louise Formhals

Charlotte Louise Formhals, 81, of Ottawa passed away Monday, November 17, 2008 at St. James Hospital and Health Center in Chicago Heights. Funeral Services will be held at 11:00 AM Thursday, November 20, 2008 at Ottawa Funeral Home with Rev. David Kipfer officiating. Burial will be in Oakwood Memorial Park. Visitation will be from 4:00 - 7:00 PM Wednesday at the funeral home. Mrs. Formhals was born April 20, 1927 in Ottawa to Victor and Elizabeth (Bonhorst) Molinoski. She married Carl Formhals July 9, 1977 in Ottawa. She was a member of the American Legion and VFW Auxiliaries. Charlotte is survived by her husband, Carl of Ottawa; two daughters, Phyllis Aldrich of Streator and Betty Weishaar of Marseilles; four grandchildren, Vicki Aldrich, Kayla and Jarrod Weishaar and Deena Johnson; three great-grandchildren, Raven Sluder, Joshua Beardsley and Dylan LaBuff and many nieces and nephews. She was preceded in death by one daughter, Gail Aldrich; two sisters, Marie Molinoski and Mollie Brown and one brother, Victor Molinoski, Jr. Pallbearers will be Josh Beardsley, Jarrod Weishaar, Dave Brewer, Bob Cupples, Joe Formhals and Dave Galletti.
